Claims FAQs

Accident and Illness related claims such as Daily Hospital Income, Disability, and Dismemberment and Fire Incident must be filed by the policyholder, or the person insured by the policy.

Death Claims may be filed by the beneficiary designated under the insured’s policy contained in the insurance contract or by their legal representatives

The waiting period refers to the time period the insured is required to wait for, from the date of issue of the insurance policy, to be able to avail the benefits under the policy. The terms and conditions of a waiting period may vary per product.

During this period, the insured is given a set length of time to pay his/her premium due for continuous coverage. Your policy cover will terminate at the end of the grace period if the premium remains unpaid.

During this specific period (from the initial activation of policy), the insured can cancel the insurance policy contract for whatever reason and get a full refund of the premium payment made.

There are three ways to report a claim:

  1. By filling up the online claim notification form via online form below or
  2. By sending an email with your policy and claim details to claims-ph@insuranceko.ph;or
  3. If you have an existing account with us, click the human icon on the navigation bar located at the upper right corner to open your profile page. Click the active plans tab and click the Claim button to open the online claims notification form. Fill up the necessary information and submit.

Once we receive your notification, an InsuranceKo representative will contact you within 24-48 hours to assist you throughout the claims process.

The following are the basic claim requirements. Soft copies of these documents may be submitted to claims-ph@insuranceko.ph.

Life Claims

  • Notice of Claim Form – forms will be provided
  • Attending Physician Statement– forms will be provided
  • Death certificate issued by Civil Registrar
  • A government issued valid ID of the insured person and the beneficiary

Accidental Death Claims

  • Notice of Claim Form – forms will be provided
  • Attending Physician Statement – forms will be provided
  • Death certificate issued by Civil Registrar
  • A government issued valid ID of the insured person and the beneficiary
  • Police Blotter Report/Incident Report

Accidental Disablement Claims

  • Notice of Claim Form – forms will be provided
  • Medical Certificate/Medical Abstract
  • A government issued valid ID of the insured person and the beneficiary
  • Police Blotter Report/Incident Report
  • Photographs if there is amputation (whole body and close shot) showing the amputated parts of the body

Hospitalization Claims

  • Government Issued valid ID of the insured person.
  • Hospital discharge certificate or any other documents issued by the hospital providing detail such as date of admission, date of discharge, summary of diagnosis and treatment given. This can be your Hospital’s Statement of Account and medical certificate from your doctor.
  • Police Blotter Report/Incident Report if due to accident

Fire and Lightning Claims

  • Notice of Claim Form
  • Bureau of Fire Protection Certification
  • Picture of the damage portion of the house

For us to verify your account, you need to provide important information such as your Complete Name and Policy Reference Number, or you may fill up the online form here.

A minor beneficiary cannot file a claim. The legal guardian is the one authorized to file the claim on behalf of the minor beneficiary with the submission of a notarized legal guardianship form/ affidavit and other pertinent legal documents.

Enrollment FAQs

You may browse for insurance products here. Read the product details, full product terms & conditions, and click agree to proceed. You will then be asked for the additional information needed for your policy and submit. Then, you’ll need to review the policy details and confirm that all data are true and correct. You will also need to agree to the website terms and conditions and data privacy policy. Finally, click buy and choose payment options and pay to complete the transaction. You will be receiving a purchase confirmation to your email from InsuranceKo and copy of your policy confirmation from the Insurer.

Yes, you will need to complete the registration/sign-up in order to proceed and purchase any insurance products in our online shop for a seamless transaction within the shop.

You will be receiving a purchase confirmation to your email from InsuranceKo upon successful purchase. The copy of your policy or insurance form from the Insurer is sent to your email 24 to 48 hours after the insurer receives your application.

You will receive a copy of your policy through email. If you wish to receive a physical copy of your policy, you may request it through email at enrollment@insuranceko.ph or by filling up our online support form here. Taxes and delivery charges may also apply.

Yes, you may assign one beneficiary to your policy who is not disqualified by law and of legal age.

EnsureMePh Insurance Broker, Inc. (operating under the brand name InsuranceKo) is a licensed insurance broker regulated by the  Insurance Commission under License No. IB-27-2022-R, with registered address at Corner Jayme- Commission Civil Sts. InsuranceKo wants to simplify insurance for Filipinos – from their understanding of it, why they should get one (or more), and how they can purchase it! InsuranceKo offers more financially attainable options for the ordinary Filipino, through an online marketplace where they can easily purchase insurance products for themselves and for others.

Simply select the product that you want to purchase from the menu of products in the InsuranceKo shop. You will be asked to provide the personal information needed for the processing of your insurance application. You will also be asked to pay the corresponding premium amount via our Payment Channels. 

You may send your inquiries, complaints, claim notifications, requests for a copy of the insurance policy or update customer information, here . Someone from our team will reach out to you via email or phone call to guide you through the claims process, or to address any inquiry or requests, and/or resolve any complaints that you may have relating to your insurance policy.

You will need to log in to your InsuranceKo account on the website to be able to view your policy details. You can check the basic information about your policy such as the product/plan you are subscribed to, the name of the Insurance Provider, your personal information, the insurance cover start date and end date, the insurance terms and conditions, and the benefit amount in the ‘Manage Policy’ tab under your profile management page.

Payment FAQs

You may pay your premium using our payment portal. You will be asked to select the mode of payment you want to use.

A failed payment notification will pop up informing you that payment has failed or did not push through. You may select other forms of payment from the list of options in the payment platform.
